Choose the kind of leave, enter the first and last day off, and the form counts the working days — skipping weekends and any company holidays you list — and turns them into hours, including a half day at either end. Add the current balance to see what's left after the request. The PDF has the employee's details, the dates, a coverage plan, a signature line and a manager section for approval, paid and unpaid hours and comments; the CSV is one row for your leave log.
The employee's name and department, the manager, the type of leave, the first and last day off, the number of working days and hours, the date it was submitted, and how the work will be covered. A reason is optional for vacation; for sick, family or medical leave keep it to what your policy needs — the form doesn't ask for medical details.
The manager's half records the decision, how many hours are paid and unpaid, and when it was entered in payroll, so the paper trail matches the timesheet.
A request from a Monday to the following Wednesday is 10 calendar days but 8 working days once the weekend is skipped — 64 hours at 8 hours a day. Take the last afternoon as a half day by entering 4 hours for the last day and it becomes 60. With a balance of 80 hours, 20 are left afterwards. The form shows a shortfall if the request is bigger than the balance.

Under the federal Family and Medical Leave Act, eligible employees of covered employers can take up to 12 workweeks of job-protected leave in a 12-month period. Private employers are covered if they have 50 or more employees in 20 or more workweeks; an employee is eligible after 12 months with the employer and 1,250 hours of service in the 12 months before the leave, at a location where the employer has at least 50 employees within 75 miles (U.S. Department of Labor, dol.gov/agencies/whd/fmla, checked 2026-10-02). Many states add their own paid or unpaid leave laws.
Choose "Family or medical leave (FMLA)" as the leave type so the request is logged separately; your HR process will need the DOL's own notice and certification forms as well.
